C12 PITTSBURGH MEMBERSHIP APPLICATION

C12 CEO Forum participants are CEOs or Business Owners with organizational revenue of over $2 Million and at least 10 employees. They believe that Jesus Christ is Lord, the whole Bible is wholly true, and God has an eternal plan for each believer’s life and that plan includes their business.

Key Players are the “right hand” person of CEOs who are members of a C12 CEO Forum.

Members meet monthly and commit to attend at least 10 monthly meetings each year. Members also schedule a 60-minute monthly meeting with the C12 Chair, primarily to discuss their “to-dos” from the previous forum.

Covenant of Mutual Benefit
a. I agree to maintain strict confidentiality, never sharing another member's business, relationships, or personal details.
b. I understand that soliciting other members for services or products is prohibited. Business relationships within C12 may occur naturally, but a C12 Forum is not a lead-sharing or networking group.
c. I affirm the C12 doctrine statement (above) and agree to refrain from divisive behavior while embracing the pursuit of God's best in all things according to the Bible.
d. I understand that membership is granted on an automatically renewable basis and that fees (which can be paid monthly, quarterly or annually) must be kept current at all times (i.e., paid prior to each monthly meeting)
e. I understand that breaching C12's stated core principals and standards - especially a. above - will result in cancellation of my membership.
f. Prior to terminating my membership for any reason, I promise to express my intention and reasons for termination to my C12 Forum members in my final meeting.
